Yes, you can modify the template standard that Netbeans makes available to any file.
In the top bar, click "Tools" and then "Templates". Select the template package you’d like to modify (in your case, PHP), then select the template/file type (new class, interface, etc) and click "Open in Editor".
Make the necessary modifications and save the new model.

I don’t use netbeans but I believe it is possible to create a default template with the desired requires.
