Visual Studio can use this directly (in fact you only need the compiler), but if you want something external that does something more sophisticated:
I would take one of the top 3. I preferred to skip the paid ones. Rate which one is best for your need.

Yes, the Sandcastle.
There is still the NDOC and the Docproject that may be useful.

Recommend Sandcastle also. It has a lot of flexibility and various transfers (.chm, .hxs, .xmshc)...although it does not create current wiki pages'.

Yes, but you open XML, I would like to open it in document form, help and if possible formatted and not with XML tags.
