I developed a site that contains a user registration, then the site administrator releases this registration, but need to add some more fields and by default Wordpress only asks for username and email, as add for example name and phone in this registration, link to the registration form

From what you’re saying, it seems to be the case mark an answer as accepted. Here we do not write "solved" in the question. If you have an answer that really helped you, mark it as accepted. If you came to the solution on your own, put in the solution as an answer. So content is more organized and easier to find in the future by other people with similar problems.

As I say, sometimes it just takes a little research.
I found a plugin that makes exactly the prentendido, called:

Cimy User Extra Fields

This plugin offers the option to add things like:

text, textarea, textarea-rich, password, checkbox, radio, drop-down, picture, picture-url, registration-date, avatar, file, date

You can read the full description of the features of this plugin and see more screenshots in:

The code of this plugin has improved a lot but is still quite raw. It has to filter, but here are some good solutions to do this by hand. Worth checking out other plugins too.
