After installing SQL Server 2008 R2, access SQL Server Management Studio but server name does not appear and SQL Server Configuration Manager services are not active.

This issue is related to an incomplete update that did not update all the components needed to run one or more SQL Server services.
Install the latest SQL Server 2008 R2 Service Pack to fix this issue.
