I have a form in PHP where presents the data of the logged in user. Follow code:

After I put Bootstrap to give it responsive, it stopped returning the data, now always presents the value 1 in the fields. I’ve already taken the test, except, changing the div that makes the responsiveness:

When I take it, the form works, presenting the data correctly.
