Duplicated data when sending form and F5 and also stores empty values

Whenever I hit F5 or click send, php stores the data again. I know it’s a common problem and I’ve tried redirecting with the header, but it’s included in index.php with include.

I also know that the words msql should have an i at the end but I will adapt later, I used php and ajax as I do to solve the problem?

Hello, 1º - The script is done to record every time q "send" is clicked. One option would be to disable send button after click. Ex: $("#enviar").prop("disabled",true);

2º - The whole part of the query is inside the if’s of the name and surname. Using a program or site that indents automatically becomes easy to see. Ex: Phpformatter, Phpbeautifier

Entering $_POST direct is not safe
