How to get a picture of the user’s computer?

I’m starting to build a form in HTML (bootstrap), and I want to add a div with an "add image" button that is possible to fetch the image inside the computer, and as soon as the photo is added, that the user can type a caption below the visualization of it. And have another button to add new images and repeat the whole process. (js)
