How can I store questions in this style:

a.  __She__ is alone in the park, sitting by__herself___. (She/Her/Herself)

in the bank with these spaces to fill and use these spaces on android for example.

You could use some specific markup to indicate the use of the "__" set and write to the base with that markup. When the recover application you could replace it with the true characters again.

Example:

#### is alone in the park, sitting by ####. (She/Her/Herself)

    These "___" is a line just to indicate where the answer is.

    Exactly. You can use markup to specify the places where values should be set. I will edit the example I suggested.
