Image with dynamic text to send by e-mail

Question

I wonder if it is possible on a page to insert a picture of a coupon on. jpg on the background and on the image of the coupon some fields like name, phone and email to fill.

After filling the user click on a send button and the image is sent as an attachment to him by the same email that he filled but with text of the data that were already filled saved in the image.
