How do I create a class diagram and database project in Visual Studio online? Or is only available for Ultimate version of Visual Studio?

Visual Studio Online does not have these features. Here is the list of resources up to the date of this reply.
These features are usually even from Visual Studio Ultimate. There is also Entity Developer Express which works in the same way for Express and Community versions of Visual Studio.
